If you were employed by legacy SCS or MCS and need a copy of your professional transcripts prior to September 1, 2016, complete the form below.

• We DO NOT have records prior to 2013.
• If you need transcripts after September 1, 2016, your transcripts can be found in the PLZ.
• If you were employed by SCS after September 1, 2016, and you no longer have access to the PLZ, please complete the form below and indicate in the Comments field that you need your PLZ transcripts.
